How to Set Up a Patient Portal Account

Upon discharge from our hospital, the email address you shared with Artesia General Hospital during the registration process will receive an auto-generated email from our electronic health record service. This email will provide step-by-step instructions on how to access your personal medical record.

Please note: our Patient Portal relates to services provided at Artesia General Hospital only and will not include health information from any other healthcare facilities that you may have used for health services.

1. Email Invite
You will receive an email invitation to create your Patient Portal Account. Click on the link provided in the email to start the registration process.

Note: You cannot initiate a Patient Portal account on your own. You will be sent an email by our system automatically after discharge from Artesia General Hospital.

2. Portal Registration
Please complete the required information to start the registration process. You will be asked to create a username and password following the set criteria. Once complete, check the “I verify” box and click “Register” to continue.

3. Select an Avatar
As a security feature, select a small “character” to represent you in the Patient Portal System. Click “Submit” to continue.

4. Security Questions
Select and answer three security questions to protect your account. Click “Submit” to continue.

5. Success!
You have successfully registered for the Patient Portal! Click the link to continue.

Once logged into your Patient Portal Dashboard, you will be able to view messages and medical records, pay bills, change settings and use the help center.

You can login to your Patient Portal Account at any time.

What is the Patient Portal?
Artesia General Hospital’s Patient Portal is an online health electronic document management tool that includes a view of clinical data from your Electronic Medical Record (EMR). Our Patient Portal allows you to:

The AGH Patient Portal and AGH Health Records are currently only available if you have been a patient since May 1, 2015.

How do I set up an account?
You cannot initiate a Patient Portal account on your own. You will be sent an email automatically when you are discharged from Artesia General Hospital. The email will be sent to the email address you shared with us during the hospital registration process. Follow the instructions to create your account.

How do I access my Patient Portal account?
After completing the set-up process, you can access your Patient Portal Account at any time.

Do I need special equipment?
You will need a computer with internet access and access to the email account that you provided during hospital registration.

Can my family and friends access my information on the portal?
Yes, but only after you have given them permission. As a patient of Artesia General Hospital, you can choose to give an authorized representative access to specific hospital visits. You will be asked for this information during the admission process.

Will I receive emails after each admission to the hospital?
No. After each admission to the hospital, a new Summary of Care document will post to your patient portal. You may access the document any time after you are discharged. Once the initial email has been sent, the patient or authorized representative of the patient will not be sent new emails with each new visit unless a portal reset is necessary.
