Not sure if you need to call or come in? If you or a loved one are experiencing a medical emergency—such as chest pain, difficulty breathing, or severe injury—please dial 911 or proceed immediately to our Emergency Department, located at the main hospital entrance (open 24/7).
For non-life-threatening illnesses like flu symptoms, minor cuts, or ear infections, you can visit our Urgent Walk-In Care at Memorial Family Practice without an appointment. For all other routine inquiries, prescription refills, or scheduled check-ups, please use the directory below to contact your provider’s office directly.
2. Before You Dial: Quick Tips
Help us connect you faster. To ensure we can assist you efficiently, please have the following ready when calling our administrative departments:
- For Billing Inquiries: Please have your account number (located on your statement) and your current insurance card on hand.
- For Medical Records: Be prepared to provide your full name, date of birth, and the specific dates of service you are inquiring about.
